The gallbladder collects bile acid that has been produced in the liver in order to digest the fat in foods. Gallstones arise when soluble substances crystallize in the bile acid. This can produce many small stones, such as biliary sand, or one or more large gallstones.

Those affected notice problems only when the gallbladder has been inflamed or when gallstones block the linked passages between the gallbladder and the small intestine. The physiology tries to transport the gallstones to the bowel, and this results in painful cramps.
